Enrollment forms are due to the SAU no later than, Wednesday, May 27th

- Important: If you are impacted by the move to the 2.0 plans (all staff except for Teachers who are the health insurance subscribers), you will automatically be switched to the equivalent plan. Example: Currently enrolled in Yellow w/Choice Fund, you will automatically be switched to Yellow 2.0 w/Choice Fund. If you want to make a change, you MUST complete an enrollment/change form.
- Elected Yellow Plan with Choice Fund or Yellow Plan 2.0 with Choice Fund?: Complete your Health Assessment between July 1st - August 31st to activate Choice Fund.
- Making Changes?: Fill out the appropriate form found below.
- Not making changes to medical, dental and/or vision?: You do NOT need to complete an enrollment form.
- Flexible Spending is an annual enrollment: You need to complete a new flexible spending account form on an annual basis if you wish to participate. This benefit is not automatically renewed!
- Health Account - IRS 2026 Maximum contribution is $3,400 ($680 rollover)
- Dependent Care Account - IRS Maximum is $7,500 per household; $3,750 per person if married and filing separately
